GRADES OF GYNECOMASTIA

The surgical options for gynecomastia depends on the severity of the condition. The severity of gynecomastia can be divided into four grades. These grades are based on how much glandular and breast tissue are present (in grams).

Grade I (< 250 grams)

Grade I is characterized by an increased diameter and slight protrusion of the breast gland but limited to the areolar and nipple region. Since this enlargement is due to the growth of the breast gland tissue without any surrounding fatty tissue, glandular excision without liposuction under local anesthesia is the best surgical approach.

Grade II (250–500 grams)

Grade II gynecomastia is characterized by moderate enlargement of the breast tissue along with the nipple and areola placed above the inframammary fold. The best line of treatment in this case is liposuction of the fatty breast tissue with high definition sculpting of the underlying chest muscle followed by surgical excision of the breast gland. The breast gland is removed through an incision that follows the border of the areola to hide the incision.

Grade III (> 500 grams)

Grade III shows a large amount of breast tissue rising off the underlying muscle with the breast gland starting to point downwards.. The nipple and areola is located at the same height up to 1 cm below the inframammary fold in this grade. The surgical procedure for grade III gynecomastia starts with liposuction followed by excision of the gland through a cosmetic incision in the border of the areola. Drains may or may not be required to remove the excessive fluid buildup that can occur after surgery. A skin reduction surgery may be required in the future if skin contraction postoperatively is not adequate.

Grade IV

Grade IV is characterized by a large amount of breast enlargement along with excessive skin. There is severe drooping of the breast. The nipple and areola can be seen more than 1 cm below the inframammary fold. The surgical approach to grade IV gynecomastia that would achieve the best cosmetic result would be a bilateral mastectomy (breast removal) with free nipple grafting. In this procedure, the entire breast fat, gland, and excess skin is excised. The nipple is removed, made smaller, and replaced back to a position on the middle chest. Drains are usually required for about 4-6 days

GYNECOMASTIA: WHAT IS IT?

Gynecomastia is derived from the Greek words gynec meaning feminine and mastos that means breasts. Literally, the term “male breast” is relative to a condition that results in excessive growth and enlargement of breast tissue in men.

The male breast consists of both glandular tissue which is composed of ductal and stromal elements, as well as adipocytes or “fatty tissue”. The excessive growth of glandular tissue results in overgrown breasts in males that are unsightly and embarrassing for men. Patients with severe gynecomastia may have excess skin leading to considerable ptosis (or sagging) of the breast.

"...now I can wear any shirts without worrying about my chest."

“My mom bought this compression shirt for me because I used to workout almost everyday. I loved the shirt however, my chest area looked puffy and pointy no matter how lean I got from the right diet and exercise so I never really liked to wear it. Thanks to X-Sculpt I couldn’t be happier with my results, now I can wear any shirts without worrying about my chest. Dr Adajar, Anabel and whole X-Sculpt team are very caring, friendly and I’d highly recommend it to anyone who’s going through the same struggle”

Male Puffy Nipple Syndrome FAQ

1.   What causes puffy nipple syndrome?

The overgrowth of the gland that leads to the puffy nipple appearance is most often due to an increase in estrogen hormone that originates during puberty. Estrogen is made from testosterone by the conversion of testosterone via an enzyme called aromatase. The sudden increase in testosterone during this puberty causes an increase in estrogen due to the conversion of testosterone to estrogen from the aromatase enzyme.  It is the high estrogen levels that causes the development and growth of the breast gland leading to puffy nipple syndrome.

2.   Can puffy nipple syndrome go away on its own?

During puberty if the testosterone and estrogen levels return back to normal before one year from the appearance of the breast gland enlargement then the gynecomastia usually resolves. However, If the breast gland remains enlarged for a year or more then often it becomes permanent.  The development of fibrous scar tissue within the gland itself occurs after about a year.  At that point, the only permanent treatment would be surgery.

3.   How is male puffy nipple surgery performed?

Surgery to treat puffy nipples permanently is usually done under local anesthesia. Because individuals with puffy nipple syndrome tend to be very lean with a low body fat percentage, liposuction is not necessary.  A local anesthetic is injected around the nipple and gland. A small incision is then made along the border of the nipple to hide the incision.  Through this incision the gland is excised. The incision is enclosed with dissolving sutures and covered with a sterile dressing. The patient goes home immediately after the procedure.

 4.   What is the recovery and downtime for male pointy nipple surgery?

Because no liposuction is required, the recovery for puffy nipples surgery is relatively quick. A compression vest is used for several weeks to limit swelling after surgery and to prevent fluid collections such as hematomas or seromas. Return to exercise can occur within two weeks. The incisions are usually fully healed by then as well. The discomfort after surgery is often quite minimal and can be managed usually with extra strength Tylenol or ibuprofen.

Every patient is different and it’s important to follow your surgeon’s instructions for the best result.

5.   Can puffy nipples in men go away without surgery?

Non-surgical treatment for puffy nipples often depends upon how long the condition has been present.. It takes about a year for fibrous scar like tissue to develop within the breast glad. Once fibrous scar tissue develops, the gynecomastia is permanent and surgery is the only treatment. However, if medical therapy is started early and aggressive enough, then the gynecomastia can shrink and in some instances disappear completely. Aromatase inhibitors that block the enzyme that converts testosterone to estrogen as well as estrogen receptor blockers are the most often used.  Your primary care doctor, endocrinologist, or a surgeon who specializes in gynecomastia would be the best resources people to help prescribe these medications if necessary.

6.   How long does male puffy nipple surgery take?

*Gynecomastia surgery takes about 45 minutes up to 2 hours to perform. This can vary depending how much fat and breast tissue has to be removed. The procedure is done as an outpatient, so you will be able to return home shortly after.

7.   How long will it take to see my results?

*You will likely see results immediately after the procedure. Swelling and bruising is common in the beginning, but usually goes away after a couple of weeks.

*As the healing process continues, you will see a flatter, more defined chest. The healing process does take time and the final result will likely be most appreciated 2 to 3 months after surgery.

*Combined with a program of a healthy diet with exercise, many patients may continue to see positive results 6 to 12 months after surgery!

"Absolutely incredible and honestly life changing results from Doctor Adajar. He really knows his stuff. Expectations were met and exceeded. Would highly recommend seeking him out if you are suffering from gynocomastia. a big thing I really appreciated was how up front and honest they were about the pricing. No hidden costs. I’d highly recommend finding a specialist for this kind of thing, as results can turn out very badly if the person doing the surgery is not an absolute expert, and there’s probably few places out there who can match what xsculpt does. The whole team was great. I don’t have a single complaint. Would give more stars if could." -Verified Google Review
