WHAT IS MALE PUFFY NIPPLE SYNDROME?

When a man’s nipples protrude significantly above the level of the skin they are often described as “puffy nipples “ or “pointy nipples” in appearance.  Puffy nipples syndrome may be an early form of Grade I gynecomastia.  Gynecomastia is a condition in males where there is overgrowth of the breast tissue and fat behind the nipples.

Male Puffy Nipple Syndrome FAQ

1.   What causes puffy nipple syndrome?

The overgrowth of the gland that leads to the puffy nipple appearance is most often due to an increase in estrogen hormone that originates during puberty. Estrogen is made from testosterone by the conversion of testosterone via an enzyme called aromatase. The sudden increase in testosterone during this puberty causes an increase in estrogen due to the conversion of testosterone to estrogen from the aromatase enzyme.  It is the high estrogen levels that causes the development and growth of the breast gland leading to puffy nipple syndrome.

2.   Can puffy nipple syndrome go away on its own?

During puberty if the testosterone and estrogen levels return back to normal before one year from the appearance of the breast gland enlargement then the gynecomastia usually resolves. However, If the breast gland remains enlarged for a year or more then often it becomes permanent.  The development of fibrous scar tissue within the gland itself occurs after about a year.  At that point, the only permanent treatment would be surgery.

3.   How is puffy nipple surgery performed?

Surgery to treat puffy nipples permanently is usually done under local anesthesia. Because individuals with puffy nipple syndrome tend to be very lean with a low body fat percentage, liposuction is not necessary.  A local anesthetic is injected around the nipple and gland. A small incision is then made along the border of the nipple to hide the incision.  Through this incision the gland is excised. The incision is enclosed with dissolving sutures and covered with a sterile dressing. The patient goes home immediately after the procedure.

4.   What is the recovery and downtime for puffy nipple surgery?

Because no liposuction is required, the recovery for puffy nipples surgery is relatively quick. A compression vest is used for several weeks to limit swelling after surgery and to prevent fluid collections such as hematomas or seromas. Return to exercise can occur within two weeks. The incisions are usually fully healed by then as well. The discomfort after surgery is often quite minimal and can be managed usually with extra strength Tylenol or ibuprofen.

Every patient is different and it’s important to follow your surgeon’s instructions for the best result.

5.   Can puffy nipples go away without surgery?

Non-surgical treatment for puffy nipples often depends upon how long the condition has been present.. It takes about a year for fibrous scar like tissue to develop within the breast glad. Once fibrous scar tissue develops, the gynecomastia is permanent and surgery is the only treatment. However, if medical therapy is started early and aggressive enough, then the gynecomastia can shrink and in some instances disappear completely. Aromatase inhibitors that block the enzyme that converts testosterone to estrogen as well as estrogen receptor blockers are the most often used.  Your primary care doctor, endocrinologist, or a surgeon who specializes in gynecomastia would be the best resources people to help prescribe these medications if necessary.

6.   How long does puffy nipple surgery take?

*Gynecomastia surgery takes about 45 minutes up to 2 hours to perform. This can vary depending how much fat and breast tissue has to be removed. The procedure is done as an outpatient, so you will be able to return home shortly after.

7.   How long will it take to see my results?

*You will likely see results immediately after the procedure. Swelling and bruising is common in the beginning, but usually goes away after a couple of weeks.

*As the healing process continues, you will see a flatter, more defined chest. The healing process does take time and the final result will likely be most appreciated 2 to 3 months after surgery.

*Combined with a program of a healthy diet with exercise, many patients may continue to see positive results 6 to 12 months after surgery!
